Geriatric Care Management
Geriatric Care Managers, also known as Aging Life Care Professionals, provide guidance, support, resources and case management to seniors. Geriatric Care Managers will hold a degree in a human service field like social work, nursing, gerontology, or counseling and will be certified through one of the three organizations for care management- Aging Life Care Association (formally National Association of Professional Geriatric Care Managers), The National Association of Social Workers, or the Commission for Case Managers.
A Geriatric Care Manager can be hired by the senior, or their family or decision maker to make regular visits, advocate for identified needs, coordinate care, accompany to physicians appointments, coach on importance of compliance with medical orders, assist with finances, encourage evolvement in social activities, and keep open communication between the senior and their family. Prices will vary depending on area and agency but will typically range from $150-$200 an hour and an initial assessment fee will usually be charged. More hours will be needed in the beginning of care but most will need about two to eight hours per month. More information on Geriatric Care Managers and resources on finding one in your area can be found at http://www.aginglifecare.org/.
